All extra-curricular participants and a Parent/Guardian must review the Student/Activity Handbook. Students are ineligible to begin participation in extra-curricular activities until all required forms are completed.
All forms should be completed through Skyward Family Access.
Except the WIAA Physical needs to be completed at your doctor's office.
